1. Sponsor
The $15,000 On Us Giveaway (the "Giveaway") is sponsored by Great Lakes Home Remodeling, 1602 W Bancroft St, Toledo, OH 43606 ("Sponsor"). This Giveaway is not sponsored, endorsed or administered by, or associated with, any professional sports team, league or venue.
2. Eligibility
The Giveaway is open only to legal residents of Michigan, Ohio and Indiana who are at least 18 years old at the time of entry. Employees, officers and directors of Sponsor, its affiliates and its advertising and promotion agencies, and the immediate family members (spouse, parents, siblings, children and their spouses) and household members of each, are not eligible. The Giveaway is subject to all applicable federal, state and local laws and regulations.
3. Entry period
The Giveaway begins at 12:00 a.m. Eastern Time (ET) on November 1, 2026 and ends at 11:59 p.m. ET on May 1, 2027 (the "Entry Period"). Sponsor's computer is the official clock for the Giveaway.
4. How to enter
There are two ways to enter. Both count the same.
Online: Visit greatlakeshomeremodeling.com/15k-giveaway-exclusive-offer-for-fans and complete the entry form with your first and last name, phone number, ZIP code and email address, then submit the form.
By phone: Call 800-715-7663 during regular business hours, mention the $15,000 On Us Giveaway and give the same information to a Sponsor representative.
Limit one entry per person and per email address, no matter which method is used. Additional entries will be disqualified. Entries that are incomplete, illegible or received after the Entry Period, or that are generated by a script, macro or other automated means, are void.
5. Prize
One Grand Prize will be awarded: a credit of $15,000 toward the purchase and installation of a roofing, siding or windows project by Sponsor. Approximate retail value: $15,000.
The prize must be applied to a residential property in Michigan, Ohio or Indiana, within Sponsor's service area, that the winner owns or that the winner occupies with the written approval of the property owner. The winner must sign a project agreement with Sponsor within 30 days after being notified of winning, or the prize will be forfeited and may be awarded to an alternate winner.
The credit applies only to the contract price of the project. If the project costs less than $15,000, the unused amount is forfeited. Any project costs above $15,000 are the winner's responsibility. The project is subject to Sponsor's standard site inspection, permitting, scheduling, installation terms and warranties. The prize cannot be combined with other Sponsor promotions unless Sponsor agrees in writing.
The prize is not redeemable for cash and is not transferable. No substitution is allowed, except that Sponsor may substitute a prize of equal or greater value if necessary. The winner is responsible for all federal, state and local taxes on the prize. Sponsor will issue an IRS Form 1099 for the value of the prize, and the winner must provide a completed IRS Form W-9 before the prize is awarded.
6. Winner selection and odds
One potential winner will be selected in a random drawing from all eligible entries received during the Entry Period, on or about May 15, 2027, at Sponsor's office in Toledo, Ohio. The odds of winning depend on the number of eligible entries received. Sponsor's decisions on all matters relating to the Giveaway are final.
7. Winner notification
The potential winner will be notified by phone and email using the information provided with the entry. The potential winner must respond within 7 days of the first notification attempt and must sign and return an affidavit of eligibility, a liability release and, where lawful, a publicity release within 14 days of receiving them. If the potential winner cannot be reached, is found ineligible or does not comply with these rules, the prize will be forfeited and an alternate winner may be selected from the remaining eligible entries.
8. Publicity
Except where prohibited by law, accepting the prize gives Sponsor permission to use the winner's name, city, state, likeness and photos of the completed project in advertising and promotion, in any media, without further compensation.
9. General conditions
Sponsor may cancel, suspend or modify the Giveaway if fraud, technical failure or any other factor beyond Sponsor's reasonable control affects its integrity, and in that case may award the prize from eligible entries received before the action was taken. Sponsor may disqualify anyone who tampers with the entry process or violates these rules. Sponsor is not responsible for lost, late, incomplete or misdirected entries, incorrect entry information, or any computer, network, phone or software malfunction.
10. Release and limitation of liability
By entering, entrants agree to release and hold harmless Sponsor and its affiliates, officers, directors, employees and agents from any claims, losses or damages arising from participation in the Giveaway or from accepting or using the prize. This release does not limit Sponsor's obligations to the winner under the project agreement or Sponsor's product and workmanship warranties.
11. Disputes
These rules are governed by the laws of the State of Ohio. Any dispute arising from the Giveaway will be resolved individually, without any form of class action, in the state or federal courts located in Lucas County, Ohio.
12. Privacy
Information collected from entrants is subject to Sponsor's Privacy Policy. Entrants may be contacted about the Giveaway and, as described on the entry form, about Sponsor's products and services.
13. Winner list
For the name of the winner, send a self-addressed, stamped envelope within 60 days after the drawing to: $15,000 On Us Giveaway Winner, Great Lakes Home Remodeling, 1602 W Bancroft St, Toledo, OH 43606.
